4. สิทธิ์: ท่านสามารถถอนความยินยอมได้ตลอดเวลา โดยติดต่อเจ้าหน้าที่
5. ระยะเวลา: เก็บรักษาข้อมูลตามระยะเวลาที่กฎหมายกำหนด

I consent to allow Mplus to collect, use, or disclose my personal data under the following conditions:

1. Purpose: For preliminary mental health assessment, risk screening, and follow-up care and support.
2. Data collected: Nickname, phone number, mental health assessment results (stress, depression, resilience).
3. Data usage: Database management, system development, follow-up care for those at risk.
4. Rights: You may withdraw consent at any time by contacting staff.
5. Retention: Data is retained as required by applicable laws.
